Chronic pain is the most common indication that brings patients to cannabis clinics. It is not about ordinary, passing pain, but the kind that lasts for months and does not ease despite standard treatment.

Below we explain which types of pain medical cannabis is used for, how it works, who qualifies and what the path to a prescription looks like. Without promising miracles - honestly.

Which types of pain it is used for

Doctors consider medical cannabis especially for neuropathic pain (resulting from nerve damage) and chronic pain that responds poorly to first-line treatment. In practice this includes, among others:

  • back pain and sciatica,
  • joint pain and degenerative changes,
  • neuropathy, including diabetic neuropathy,
  • pain in cancer,
  • post-shingles pain (neuralgia).

    How it works and what to expect

    Cannabinoids (mainly THC and CBD) act on the endocannabinoid system, which is involved in how we feel pain. In some patients this helps reduce pain intensity, improve sleep and daily functioning.

    It is important to have realistic expectations. Medical cannabis is not a miracle cure and will not work the same for everyone. It is most often a supportive therapy, added to existing treatment rather than replacing it. Whether it is justified in your case is decided by the doctor.

    Who qualifies

    Medical cannabis is not a first-line medicine. The doctor considers it when standard pain treatment does not work or is poorly tolerated. At the visit they assess:

  • the nature and duration of the pain,
  • previous treatment and its effectiveness,
  • current medication and possible interactions,
  • contraindications (for example certain heart conditions or mental disorders).
  • There is no prescription guaranteed in advance - the decision depends on the medical assessment. That is normal and a sign of a reliable clinic.

    How to start therapy

    Therapy starts with a small dose, increased gradually (the start low, go slow rule). This makes it easier to find an effective dose with the fewest possible side effects.

    The doctor chooses the form (flower, oil, extract) and the THC to CBD ratio for the specific problem. After some time it is worth coming back for a follow-up visit to assess the effect and adjust treatment.

    How to get a prescription

    The path is simple: you book an in-person visit with a doctor who assesses the indications and, if therapy is justified, issues an RpW e-prescription valid for 30 days. You buy the flower or extract separately at a pharmacy.

    What happens during the consultation

    The visit is in person - at CannabisMedic all consultations take place at the clinic, in Krakow, Bydgoszcz, Toruń or Kielce. It takes around 10 minutes, because it serves a medical assessment, not a lecture on cannabinoids.

    The doctor goes through the following with you:

  • the history of your pain - how long, where, what kind, what makes it worse,
  • previous treatment - which medicines, at what doses, with what result and what side effects,
  • other conditions and preparations you take - for interactions and contraindications,
  • your expectations - less pain, better sleep, cutting down opioids, getting back to work.
  • If the indications are there, you leave with an e-prescription and a plan: which preparation, what starting dose, when to come back. If they are not, you will hear why and what can be done instead.

    What to bring to the visit

    The more documentation you bring, the less time goes on establishing things the doctor has to learn anyway. Useful:

  • ID document - needed to issue the e-prescription,
  • pain treatment records - hospital discharge notes, specialist opinions,
  • imaging results - MRI, CT, X-ray, if you have them,
  • a list of your medication with doses, supplements included,
  • a note on what you have already tried - which painkillers failed or were poorly tolerated.

    How long the road to a prescription takes

    Booking - online, the slot is locked as soon as you pay.

    The visit - around 10 minutes, in person.

    The e-prescription - issued during the visit or right after it. You get the code by text message and it is also visible in your Patient Internet Account.

    The pharmacy - how fast you fill it depends on availability. Sometimes it takes calling a few pharmacies or ordering in advance.

    Follow-up - after the first stretch of therapy, to assess the effect and adjust the dose.

    These are two different bills. The visit covers the medical consultation. The medicine you buy separately at a pharmacy and it is not reimbursed by the Polish NFZ - we explain the reimbursement status in a separate article.

    When the doctor will hold back the prescription

    A refusal or a postponed decision is not rare and does not mean you did anything wrong. The most common situations:

  • pregnancy and breastfeeding,
  • past or current psychotic disorders, including a family history in that direction,
  • unstable cardiovascular disease,
  • active substance addiction,
  • being under 18 - outside exceptional cases led by a specialist,
  • no diagnosis of the cause of pain - if the pain has never been investigated, what is needed first is a diagnosis, not another medicine.
  • The doctor may also ask you to supplement your records and return to the decision later. That is not a permanent refusal.

    Safety
