When you hear "medical cannabis extract", you probably picture a single product. In reality it is a whole group of preparations that differ in route of administration, concentration and purpose. Some are inhaled as vapor, others are taken orally.
In this article we sort out the types of extracts available on prescription in Poland, explain how they differ and help you decide which one to discuss with your doctor.
All extracts are made from the same registered raw material - cannabis flower - but as compounded medicines prepared in the pharmacy they take different forms. The simplest way to divide them is by route of administration:
We cover each of them below. If you only want details on one, jump straight to the relevant section.
Extractum purum is a purified extract for vaporization, usually at 80% THC. The patient receives it in a ready cartridge that clicks into a reusable vape pen - the device heats the extract without burning it, so it produces vapor, not smoke.
Its advantages are a precise, standardized THC dose, high efficiency and discretion. It is a good option for people who value convenience and a weaker smell than flower. Everything about this form - the price, how to use it and how to get a prescription - is covered in our separate guide on extractum purum 80% THC.
Extractum normatum is most often an oil for oral use with a normalized, predictable cannabinoid concentration. It is taken sublingually (a drop under the tongue) or swallowed, and the effect appears more slowly than with vaporization but lasts longer.
This form can be convenient for people who do not want to inhale anything, or who need steady effects spread over time - for example in chronic pain or sleep problems. The choice of form is always made by the doctor based on your needs.
This is the most common misunderstanding. THC oil (extractum normatum) is a prescription medicine containing THC - a psychoactive substance with a therapeutic effect. CBD oil from a shop or the internet is a supplement without THC (or with trace amounts), available without a prescription and with a much weaker, different action.
If a doctor recommends medical cannabis therapy, they mean a prescription product from the pharmacy, not a CBD oil from a drugstore. We write more about this difference in the article on medical cannabis vs store-bought CBD.
An extract is not a "better" version of flower, just a different form of the same therapy. The choice depends on what matters to you: price, convenience, discretion or how it works.
| Criterion | Dried flower | Extract (purum/normatum) |
|---|---|---|
| Price per gram | Lower | Higher |
| Dosing | In grams | In mg of THC (standardized) |
| Route | Vaporization | Vaporization or oral |
| Smell | Strong | Weaker or none (oil) |
| Onset | Fast | Fast (purum) / slower (oil) |
| Discretion | Lower | Higher |
You do not have to choose blind. At the visit, describe your symptoms and lifestyle, and the doctor will suggest the form that fits best - and change it later if needed.
Every extract - for vaporization or oral - is a compounded medicine dispensed only on prescription. The path is simple:
We describe it step by step in our guide on how to get a medical cannabis prescription. Before you head to the pharmacy, also check where to buy medical cannabis - not every pharmacy compounds every extract.
What are the types of medical cannabis extracts?
The main division is into vaporization extracts (extractum purum, usually 80% THC, in a vape pen cartridge) and oral extracts (extractum normatum, an oil with a normalized concentration). Separately there are store-bought CBD oils, which are not a prescription medicine.
How does extractum purum differ from extractum normatum?
Extractum purum is a pure extract for vaporization that you inhale as vapor. Extractum normatum is usually an oral oil with a normalized concentration that you take sublingually or swallow. They differ in route of administration and onset.
Is pharmacy THC oil the same as store-bought CBD oil?
No. THC oil (extractum normatum) is a prescription medicine containing the psychoactive substance THC. Store-bought CBD oil is a supplement without THC or with trace amounts, available without a prescription and with a different, weaker action.
Extract or flower - which is better?
Neither form is universally better. Flower is cheaper per gram, the extract is more convenient, more discreet and more precise in dosing. The choice depends on your needs and the doctor's recommendation.
Do you need a prescription for an extract?
Yes. Every medical cannabis extract is a compounded medicine dispensed only on prescription, issued by a doctor after a consultation.
Are medical cannabis extracts reimbursed?
No. Medical cannabis and its extracts are not reimbursed by the Polish NFZ - the patient covers the cost of therapy themselves.
