
Until recently, a patient with a medical cannabis prescription left the pharmacy with dried flower and nothing else. Now, in selected pharmacies, there is a second option: extractum purum, a pure, purified extract made for vaporization, most often at 80% THC.
It is the same active substance as in flower, only in a more concentrated, more convenient and more discreet form. Below we explain what extractum purum actually is, how to use it, what it costs and how to get a prescription for it.
Extractum purum is a compounded medicine - the pharmacist prepares it from a registered pharmaceutical raw material, meaning the same cannabis flower available on prescription. During extraction the cannabinoids (mainly THC) are drawn out of the plant and then purified of chlorophyll, waxes and plant residue.
The result is a thick, resinous substance with a high THC concentration - usually 80%, though 40% variants exist too. The word "purum" (Latin for pure) refers to exactly this purification. The patient receives the extract in a ready cartridge, which clicks into a reusable inhalation device (a vape pen).
Importantly, the amount is not measured in grams of flower but in a fixed, lab-tested THC content. The pharmacy standardizes the product, so you know exactly how much active substance you are taking - a real difference from flower, where potency can vary between batches.
The extract and the flower contain the same cannabinoids, but they differ in form, use and convenience. In short: flower is cheaper per gram and more "raw", the extract is more expensive but cleaner, more efficient and more discreet.

For many patients the deciding factor is exactly this discretion and convenience - a vape pen fits in a pocket, leaves no strong herbal smell and lets you take a dose in seconds. If you are still choosing the form of your therapy, it is worth discussing with the doctor during your first visit.
Extractum purum is used through vaporization - the device heats the extract to a temperature that releases cannabinoids and terpenes without burning it. There is no smoke, only vapor.
The "start low, go slow" rule matters even more with an 80% extract than with flower, because a single puff delivers more THC. If THC hits too hard, do not panic - it is unpleasant but it passes. We covered this in a separate dosing guide.
The price of extractum purum is higher per gram than flower, because of the labour-intensive extraction and purification. But you should look at it differently: thanks to the high 80% THC concentration the product is very efficient, so one cartridge lasts longer than the equivalent amount of flower.
