If you have been taking opioid painkillers for a while and are wondering about medical cannabis, that is a good and important question. Many of our patients come to us precisely with the idea of cutting down on strong painkillers - but it is a topic that calls for caution, not hasty decisions.

Let us calmly explain what is known and how it works in practice.

Why patients look for an alternative

Opioids can be effective, but they come at a cost: the body gets used to them, higher doses are needed over time, and on top of that come constipation, drowsiness, low mood and a real risk of addiction. For some people with chronic pain this is a reason to look for something that lets a lower opioid dose be enough.

What research says

In some patients medical cannabis supports pain control enough that the opioid dose can be safely reduced - in the literature this is called an opioid-sparing effect. It is not a rule that works for everyone and does not mean cannabis will always replace opioids.

Treat it as a possibility worth discussing with a doctor, not a promise. Every case of chronic pain is different.

What you must not do

Never stop opioids on your own or abruptly. Sudden withdrawal risks a serious withdrawal syndrome and a return of pain.

Any change - lowering the dose, combining with cannabis, stopping - must happen under a doctor's supervision and gradually. This is a matter of safety, not a formality.

How it works in practice

The doctor first assesses your pain, previous treatment and your whole health situation. If there are indications, medical cannabis is introduced carefully and gradually while the response is observed. Only once it works can a slow reduction of the opioid dose be considered - always together with your treating doctor.
