
THC and CBD are the two most important compounds in cannabis. They sound similar but do something completely different: one intoxicates and relieves pain, the other calms without causing a high. Understanding this difference is fundamental, because it decides which preparation the doctor picks.

THC (tetrahydrocannabinol) is the psychoactive compound - responsible for the intoxicating effect associated with marijuana. In medicine, though, something else matters: THC relieves pain, improves sleep, stimulates appetite and suppresses nausea.
It also has side effects: in some people drowsiness, dry mouth, sometimes unease at higher doses. And crucially - THC precludes driving. That's why the dose is set carefully.
CBD (cannabidiol) doesn't intoxicate. It doesn't cause a high or impair you the way THC does. It has anti-anxiety, anti-inflammatory and anticonvulsant effects (in drug-resistant epilepsy as the medicine Epidiolex).
In cannabis therapy CBD plays one more important role: it softens THC's unwanted effects. Adding CBD can dial down the unease or racing heart that THC alone triggers in sensitive people.
THC and CBD don't work solo. Together with terpenes and other plant components they create the so-called entourage effect - the whole works more effectively than a single, isolated compound.
That's why medicine more often reaches for full-spectrum preparations rather than pure THC or pure CBD. The ratio is matched to the therapy goal.
